My two sisters and two brothers-in-law were worried about leaving our frightened and delirious 90-year-old mother in anyone’s care. She was recovering from surgery after breaking her hip and was already struggling with Parkinson’s and Dementia. Her condition was fragile and we were uncertain of where to put her for recovery, but received a recommendation for the rehab at Centennial from a trusted Geriatric Care Manager. After some research, we elected to transfer our mother to this facility. The team provided the expected services–daily Physical Therapy, Speech Therapy and Occupational Therapy. All of that was excellent and our mother made some very positive progress. But what really impressed us, was how the nurses and aides worked as a cohesive team to create a positive and loving community. The energy at Centennial was powerful, uplifting and infectious and social. Sure, most of the patients are in wheelchairs struggling with age related illnesses or injuries, but they were listening to music, hanging out together in the common areas, eating Italian ices, attending musical performances, bingo, crafts and even a carnival! While our mother’s dementia prevents her from participating in most activities, the aides took the time to bring her to these events which my mother enjoyed being a part of. There was always a helping hand to get my mother’s nails cut, wash her clothing, get us information about her medications, how much she was eating or progress in PT. Centennial has no fancy façade. It is not about appearances. It is about a building a strong community and providing absolutely excellent care. For that, the team at Centennial deserves our highest recommendation.
Everyone was very nice to my father… They give good care and actually care about their patients and their well-being and want them to become better with therapy and not just send them home if they’re not getting better. There are a lot of activities there and things to do. Well staffed.
5 stars for Centennial staff making a new long-term care resident and their family feel so cared for and welcome on Day 1. From Cynthia the SW, Littles in Admissions, Eleanor Unit Mgr, Nurse Ed and other staff members making this patient and family feel safe in their care. So wonderful to see!
Overall, the facility and amenities were clean, bright, spacious, and airy. The staff was welcoming, warm, and pleasant. Thank you for taking care of my dad, E. Haley.
My husband and I would like to thank Mercedes for her wonderful attitude and personality with the family and residents. She deserves some type of recognition for who she is and how she represents Centennial Healthcare.
As a retired Nurse and health Care Supervisor, I know a great, caring and wonderful person that exemplifies her duties and just being a caring person. The residents really seem to respond well to her. Thank you Mercedes for such a wonderful experience while visiting my father, Clifford Brown.

I came to Centennial for wound care and physical therapy. I feel great now, a lot stronger and healthier. I am able to do a lot more for myself thanks to the rehab team. I actually enjoyed my time here. I was able to build great relationships with staff and fellow residents. My activity aide kept me active doing arts & crafts, decorating masks, playing bingo, listening to music, and just making my stay pleasant!
The staff seems friendly and warm, like family.

The staff at Centennial was phenomenal! They exceeded our expectations for my dad. My dad came in to the facility on a trach, oxygen and a peg tube. He couldn’t go to the bathroom or eat. The respiratory and speech therapists were very compassionate and very patient. They worked closely with my father and he is now off the trach and the oxygen, breathing on his own and eating. The nursing staff assisted him with his needs and made sure he got to activities. My dad came with little life and now he is able to do so much for himself walking and talking! If you need a place for rehab, I recommend Centennial they have given us life again!
